Warranty Activation
Your iVox electrolarynx warranty must be activated within 4 months of original purchase date by the original owner. To activate, make a copy of and complete the information on the "Warranty Registration" pannel in your Owners Manual. Mail this information to: Archer Medical Devices LLC, Warranty Registration, PO Box 24306, Lexington KY 40524.

In the event your Owners Manual has been lost a replacement booklet may be obtained by emailing: Contact@ArcherMedical.com. Include your Name, Address, iVox Serial Number and Original Purchase Date/Distributor.

All instruments returned for warranty service must be sent to Archer Medical Devices LLC, Warranty Service, PO Box 24306, Lexington, KY 40524.

- Contact Information for Return Shipping; Mailing Address, Email, Phone Number
- iVox Serial Number

iVox Electrolarynx

2-Year Limited Warranty

·   We, at Archer Medical Devices LLC, warrant the iVox electrolarynx to be free of manufacturer defects for a period of 2 years from the date of original purchase. Included accessories, such as batteries, charger and oral-adaptor, are warranted for a period of 60 days. Archer Medical Devices LLC shall not be held responsible for any incidental or consequential damages. This warranty covers repair or replacement only. Repurchase is not a covered option.

· Limited coverage is provided for damage due to accidental drops or water immersion. An accidentally damaged iVox electrolarynx will be repaired or replaced a maximum of 3 times during the warranty period. Accidentally damaged devices must be returned immediately with all parts and pieces and include a note describing how the damage occurred. Coverage does not extend to missing parts or pieces.

·   This warranty does not cover damage caused by abuse, negligence, excessive soiling or intentional acts. This warranty will be voided by tampering, disassembly, chemical or salt-water damage and uses inconsistent with those described herein. Warranty valid only within the United States of America. All other warranties not explicitly stated are excluded. This warranty is non-transferable.

·   To validate warranty, complete and return a copy of the attached “Warranty Registration” panel within 4 months of purchase with a copy of the original purchase receipt from an authorized distributor.

·   Portions of this warranty may not be allowed in all jurisdictions. Allowable portions of this warranty will remain valid and in force within those jurisdictions.

·   Repair or replacement by new, used or refurbished parts and/or devices will be at our discretion and will be based on the age and condition of devices received. Returned devices may show signs of repair.

·   Include your name, address, contact phone number and/or email address with all devices. You are responsible for delivery charges to Archer Medical Devices LLC. Return shipping will be provided only within the United States of America.
